New Patient Checklists

At Lucid Force Health Center, it is our goal to Empower people with the knowledge and support they need to heal chronic health problems. To start you on your Path To Health, please read about what kinds of conditions bring people to us, our history, and our mission to serve patients at the highest level:

Did you remember to?

  • Read all of the practice documents.
  • Obtain your last two years of lab results, imaging studies, and/or medical records and have them sent to us at 1317 W Foothill Blvd., Suite 135
    Upland, CA 91786. You may have them faxed to: (909) 949-1998. Please make sure that we receive your past records at least 7 days prior to your appointment.
  • We will need a street address where you can receive packages sent via UPS or FED EX. Unfortunately, deliveries to PO Boxes will not be eligible for deliveries from some carriers.

    • Make sure to review our fee schedule for the appropriate service:

  • Case Review Consultation: $195
  • Cost of laboratory tests can range from $300 to $3,000 and can be prioritized for you.
  • New Patient Enrollment Fee: $295.00
  • Initial Nutrition Consultation: $295.00
  • 1st Nutrition Follow-up 50-Minutes: $250.00
  • Office Visit or Phone Follow-up 50-Minutes: $250.00
  • Office Visit or Phone Follow-up 25 Minutes: $150.00
  • Nutrition Office Visit or Phone Follow-up 50-Minutes: $250.00
  • Nutrition Office Visit or Phone Follow-up 25-Minutes: $150.00

How We Help People Get Better @ Lucid Force Health Center (LFHC)

Healthy diet and lifestyle choices lay the foundation for good health and recovery from all disease.
