About Lucid Force Health Center

At Lucid Force Health Center in Upland, CA, our goal is to help you find Your Path To Health. The information and videos on our website are to designed to educate you on your role in recovering your health, as well as healing your body. We also want you to understand the different ways in which we help you improve your health which are not utilized by your current treating doctors. We help you address chronic health concerns by addressing the causes of illness, rather than just suppressing the symptoms. Please know that you are a very important part of your recovery and through teamwork with our office, you can get your life and health back.

LFHC Practice Policies

Our goal at Lucid Force Health Center is to serve your health recovery needs at the highest possible level. Remember, labels are for cans not for people. We are very effective at helping people recover because we focus on the person, not their disease label.

It’s important to read all the information that was sent to you carefully. All forms must be completed and mailed or faxed to our office at least 7 days prior to your appointment. Additionally, any previous lab work should be sent into our office with your completed paperwork. This will allow Dr. Robles to streamline his questions in order to narrow down what may be causing your problems.

Medical Records

You are responsible for obtaining previous medical records from other physicians or health care providers. Your medical records can only be released with your authorization. A medical records release form is enclosed for your use. Please contact your physician or other health care provider to obtain these records. Your records should be mailed to our address below or faxed to (909) 949 1998.

Case Review Consultation

Your Case Review Consultation will include a consultation with Dr. Robles, a review of 2 of your most recent lab tests, a report of findings and care plan recommendations. The goal of the Case Review Consultation is to determine the best course of care for your condition. Additionally, we will discuss reasonable expectations for results based on your compliance with our recommendations.

Initial Visits

For out of town patients, it is recommended that you fly into one of our 3 local airports and stay nearby the office. This will allow us to schedule your initial visit and any necessary follow up appointments and labs prior to your departure (1-2 visits after the Case Review Consultation may be recommended based upon the severity of your condition). Additionally, if labs are performed locally, many require a fasting period of 8-12 before testing.

Case Review Consultation: $195
Cost of laboratory tests can range from $300 to $3,000 and can be prioritized for you.

1st Nutrition Follow-up 50-Minutes: $250.00

Confirmation and Cancellation of Appointments

Due to the volume of requests for Case Review Consultations, there is a 7-day cancellation policy, and your appointment must be confirmed at least 7 days prior to your scheduled consultation. You may cancel your appointment by calling our office. Please leave a message if you are calling after our business hours.

Payment Options

Lucid Force Health Center accepts cash, check or credit card (MasterCard, Visa, Discover, AMEX) for scheduled services and services rendered. When you schedule your Case Review Consultation, we request a credit card on file to hold the appointment for you. No charges will be applied to your credit card unless you miss or cancel an appointment without proper notice. On the day of your scheduled appointment, all charges for services, as well as laboratory testing will be itemized and reviewed with you. Payment is due on the day of service or for ordering lab tests.

Follow-up virtual appointments (phone or video) will be billed to your credit card on file with our office unless you provide other payment information and instructions prior to your appointment. If additional lab tests are required and our office sends test kits, the appropriate fees will be charged to your account prior to sending any test kits.

Insurance Information

We do not accept insurance or Medicare, and we do not file insurance paperwork on your behalf. Due to the nature of insurance coverage, some of the services we provide may not be deemed medically necessary or may be classified as experimental. Therefore, we cannot assure you that services (office visits, phone consultations or lab tests) will be reimbursed by your insurance carrier. You will be provided with diagnosis and procedure codes to assist you with possible insurance reimbursement at your request.
